A criminal history search of our records was performed using the information provided in the correspondence, which included the President/Stockholder's name and Social Security Number, and no records were found. Please note that a copy of Form 4030 (Stockholder Disclosure) is on file.
Timing Considerations
The Clerk of Council must respond to the Ohio Division of Liquor Control no later than August 27, 2012 concerning a request for a hearing.

Based on the above criteria alone, no hearing will be requested at this time.
